Eligibility for one service does not automatically establish coverage for every in-home visit, dressing, device, or advanced treatment.
Depending on the situation, care may be billed as a practitioner service, a home-health service, pharmacy benefit, laboratory service, or durable medical equipment and medical-supply benefit. Each pathway has its own order, provider, documentation, and authorization rules.
Most members receive services through a Medi-Cal managed care plan and should use the plan's network and authorization process. Fee-for-service Medi-Cal follows state provider-manual requirements. Dual-eligible members generally have Medicare billed first for Medicare-covered services.
Records commonly include diagnosis, wound measurements and photographs when appropriate, skilled need, prior treatment, frequency and duration, homebound status when the home-health benefit requires it, and product-specific justification.
Coverage depends on current eligibility, medical necessity, plan, network, authorization, documentation, and coordination with other coverage. Even an authorization is not a guarantee of payment. Members can request a written decision and use plan grievance, appeal, or State Fair Hearing rights when applicable.
