Mobile Wound Care in Azusa

Azusa sits at the base of the San Gabriel Mountains where the San Gabriel River comes out of the canyon, a foothill city built along Azusa Avenue and now anchored by the Metro A Line stations and Azusa Pacific University. It is a predominantly Latino, working-family community where diabetes runs above the California average, and diabetic foot ulcers are the wound our clinicians see most often here. Our licensed physicians and nurse practitioners cover the whole city: the foothill neighborhoods of North Azusa and the newer Rosedale and Mountain Cove developments up toward the canyon, downtown Azusa along Azusa Avenue, the neighborhoods around Azusa Pacific, and South Azusa toward the 210 freeway. Azusa's older sections rise into the foothills with steep streets and split-level homes, and for a patient trying to stay off a diabetic foot wound, even getting down the driveway to a car for a weekly clinic visit can be a real problem. Azusa is a heavily Spanish-speaking community, and in many households the family members providing daily wound care speak Spanish at home. Our coordination team arranges interpreter support when scheduling so offloading instructions, dressing-change technique, and the warning signs of infection are clearly understood by the patient and the relatives helping day to day. We coordinate with discharge planning teams at Emanate Health Foothill Presbyterian Hospital, Emanate Health Inter-Community Hospital, and Kaiser Permanente Baldwin Park so patients leaving the hospital with a surgical wound, pressure injury, or diabetic ulcer transition to physician-led care at home without a gap. Wound measurements, photographs, and encounter notes go back to the discharging team and the primary care physician after every visit. Visits are typically covered under Medicare Part B, Medicare Advantage, and most commercial plans. Our coordination line at 877-545-1300 is open 24/7. Call to arrange an assessment.
