Mobile Wound Care in Jurupa Valley

Jurupa Valley is one of California's newest cities, incorporated in 2011 by stitching together the longtime unincorporated communities of Mira Loma, Glen Avon, Pedley, Rubidoux, and Sky Country along the Santa Ana River northwest of Riverside. It's a working, largely Latino community with a diabetes burden running around 11% — above the state average — and diabetic foot ulcers are the wound our clinicians see most often here. Our licensed physicians and nurse practitioners see patients across all of these communities — the warehouse-and-residential blocks of Mira Loma near the 15, the established neighborhoods of Glen Avon and Pedley, the historic community of Rubidoux at the base of the hills, the Sky Country area, and the streets along Mission Boulevard. Jurupa Valley has no hospital of its own and is spread across the 60 and 15 freeway corridors, so a patient who needs weekly wound care faces a real trip into Riverside every time — exactly the barrier that lets a diabetic foot ulcer worsen between appointments. We remove it by bringing the physician to the living room. Because the city has no hospital, our patients typically discharge from the Riverside hospitals just to the south. We coordinate with the discharge planning teams at Riverside Community Hospital, Kaiser Permanente Riverside, and Parkview Community Hospital so patients leaving with a surgical wound, pressure injury, or diabetic ulcer transition to physician-led care at home without a gap in treatment. Wound measurements, photographs, and encounter notes go back to the discharging team and the patient's primary care physician after every visit. Many Jurupa Valley households are Spanish-speaking, and our coordination team arranges interpreter support when scheduling so offloading instructions, dressing-change steps, and infection warning signs are understood by the patient and the family members providing daily care.
