Ventura — officially San Buenaventura — is the county seat and a coastal city that stretches from the historic downtown around the mission and the pier out to the newer subdivisions of East Ventura. Ventura has two established hospitals: Community Memorial Hospital near downtown and Ventura County Medical Center, the county's public hospital. We coordinate with discharge planning teams at both so patients leaving with a surgical wound, pressure injury, or non-healing ulcer transition to physician-led care at home without a gap. Wound measurements, photographs, and encounter notes go back to the discharging team and the primary care physician after each visit. Many Ventura households are Spanish-speaking, and our coordination team arranges interpreter support at scheduling so offloading instructions, dressing-change steps, and infection warning signs are understood by the patient and the family.
