Santyl (collagenase) is a prescription enzymatic debrider that works gradually, while sharp debridement removes devitalized tissue with an instrument during a clinical encounter. They are not interchangeable in every situation and are sometimes used sequentially or together under a clinician's plan.
Sharp debridement can remove a larger amount of loose or nonviable tissue immediately and lets the clinician reassess depth. Collagenase selectively digests denatured collagen over repeated applications, so progress is usually gradual and depends on correct daily use and moisture balance.
Sharp debridement may be chosen when rapid source control or wound-bed visualization is needed and perfusion and bleeding risk are acceptable. Enzymatic debridement may suit patients who cannot tolerate aggressive bedside treatment or need continued debridement between visits. Stable, dry ischemic eschar should not be debrided without vascular assessment.
Santyl should be used according to its FDA-approved labeling and prescription instructions. Some metal ions and certain antiseptics can reduce enzyme activity; compatible cleansing and dressings should be selected by the treating clinician. Spreading redness, systemic illness, crepitus, or rapidly worsening pain needs urgent evaluation rather than slower enzymatic treatment alone.
A drug plan may cover collagenase when prescribed and medically necessary, but formularies, quantity limits, prior authorization, pharmacy network, and cost sharing differ.
