Provider Requirements

• Board certification in relevant specialty (Wound Care, Podiatry, Vascular Surgery, etc.)

• Medicare provider number and ability to bill Medicare Part B

• Experience in wound care or willingness to complete training

• Active state medical license in good standing

• Professional liability insurance (minimum $1M/$3M coverage)
